At Montgomery Kidney Specialists, we not only help our patients manage their chronic kidney disease, but we also help our patients control their blood pressure. High blood pressure, or Hypertension, is the most common chronic disease and a major risk factor for cardiovascular disease, stroke, kidney failure, and other diseases. Uncontrolled high blood pressure can cause the arteries around the kidneys to narrow, weaken or harden. The RPM program is covered by most major insurances. Ask your provider about participating in the RPM program at this office.

That is why we offer Remote Patient Monitoring (RPM) for patients with kidney disease and high blood pressure. Remote patient monitoring allows us to monitor you blood pressure from the comfort of your home. This allows us to manage acute and chronic conditions and it cuts down on patients' travel costs to your provider's office.

Remote blood pressure monitoring can help monitor patients with a greater risk of developing hypertension, including:

  • Individuals age 65+
  • Pregnant patients
  • Individuals with a family history of high blood pressure
  • Cancer patients

The remote patient monitoring system is easy to use. Our office will provide patients with an FDA-cleared blood pressure device for use at home and requires no setup. The data from the device is transmitted by use of cellular towers to our monitoring staff for reviewing. From there, we can analyze your readings and adjust medications, if necessary, without having to come into the office. We can also be alerted in real-time when a patient's blood pressure readings move out of the patient’s ideal range.

 

The Comprehensive Kidney Care Contracting (CKCC) Options are a set of three voluntary payment options under the Kidney Care Choices (KCC) Model designed to reduce Medicare expenditures while preserving or enhancing the quality of care furnished to beneficiaries with late-stage chronic kidney disease (CKD) and end-stage renal disease (ESRD). The CKCC Options build on the design of the existing Comprehensive End-Stage Renal Disease Care (CEC) Model and of the new Direct Contracting Models

Montgomery Kidney Specialists now offers our patients Comprehensive Kidney Care Contracting (CKCC) workshops to help educate our patients with chronic kidney disease. Patients who meet the criteria will be eligible for alignment to these options:

  • Medicare patients with CKD stages 4 and 5
  • Medicare patients with ESRD receiving maintenance dialysis
  • Medicare patients who were aligned to a KCF practice or KCE by virtue of having CKD stage 4 or 5 or ESRD and receiving dialysis who then receive a kidney transplant

These models are designed to incentivize better management of kidney disease. The goal of this model is to create better outcomes, improve quality, and reduce costs for patients with chronic kidney disease (CKD) stages 4 and 5 and end-stage renal disease (ESRD) by supporting nephrologists to promote the following practices:

  • Slowing the progression of kidney disease
  • Opting for transplantation instead of dialysis when appropriate
  • Preparing patients for dialysis if need be
  • Implementing the shift towards home-based treatment as outpatients as opposed to inpatient hospitalizations

The Kidney Care Choices Model (KCC) was developed by the Centers for Medicare and Medicaid Services in collaboration with health care providers, community organizations, patients and their families, donors, and other stakeholders.
